Save time at your first appointment! Download the appropriate set of
forms, fill them out, and bring them with you.
Your counselor is looking forward to seeing you at HopeWorks. To save
time and help make the most of your first session, please fill out forms for your first appointment. Either arrive 15 minutes
early and fill them out in our reception area, or download them from this
site and bring them with you. At your first appointment, you will also review
your rights and your responsibilities as a client of HopeWorks with your counselor.
One of those rights is that you can be assured that we will follow state law in regard to keeping
information that you share with us confidential.
1. Open the appropriate set of forms.
2. Print them on your printer.
3. Fill them out and bring them to your first appointment.
The forms require the free program Adobe Reader, which is already installed on most computers.
